Encyclopedia of Complementary and Alternative Medicine

There are many books on alternative medicine, but not many can be considered quick reference guides. This encyclopedia has 400 entries covering terms, organizations, and personalities related to the field.

Entries vary in length from one-sentence definitions to multipage essays. The shorter entries can be frustrating, and some of the longer ones, less than complete. One example is the entry Bates Method for improving eyesight. There is a brief explanation of the relaxation system and of Bates' credentials but no description of the exercises. Some entries on individuals give little or no information about their relevance to the field of alternative or complementary medicine.

On the other hand, the Yoga article provides in-depth coverage of the discipline, giving a brief history, the basic principles involved, and descriptions of the 26 branches of yoga. Another article that would have benefited from the same treatment is ayurveda. A description is given, but there are no cross-references to related entries (Chakra, Doshas) that provide additional details. The lack of illustrations is limiting; even line drawings of things like acupuncture pressure points would have added value to the book. The appendixes include a list of professional and lay organizations, a list of herbs used in herbal treatments, and a time line of alternative and complementary medical history.

That research projects involving nontraditional medicine are being conducted at prestigious medical schools illustrates the growing awareness and acceptance of holistic treatment methods. As a result libraries will be handling more questions related to the area of alternative medicine. Despite its cursory coverage of some topics, the encyclopedia provides a starting point for further exploration. Public libraries may find this useful, but libraries serving a professional audience will find it lacking.

There have been gratifying advances in the treatment of cardiac arrhythmias during the past 20 years. Whereas in the past, pharmacologic therapy was the only real option for most patients with cardiac arrhythmias, the advent of radio-frequency catheter ablation, the implantable cardioverter-defibrillator, and advances in cardiac pacemakers have substantially increased the number of options for the management of arrhythmias. It is important that general cardiologists and primary care physicians stay abreast of the main advances in the field of cardiac electrophysiology and know what treatments are available and when to refer patients to electrophysiologists for optimal therapy. The goal of Management of Cardiac Arrhythmias is to inform general cardiologists and primary care physicians about these advances in the diagnosis and management of cardiac arrhythmias. This book covers the major areas of interest in electrophysiology. The introductory chapters on the history of electrophysiology and the diagnosis of arrhythmias are followed by chapters on the diagnosis and treatment of supraventricular tachycardias, including atrial fibrillation and flutter, syncope, and permanent pacing, as well as several chapters on ventricular arrhythmias. These chapters include insights from clinical trials on implantable cardioverter-defibrillators, pharmacologic therapy, and catheter ablation. The final chapters cover miscellaneous topics related to ventricular arrhythmias, such as the Brugada syndrome and the long-QT syndrome, arrhythmias during acute myocardial infarction, and arrhythmias in children and during pregnancy. The book is very well written. Algorithms for evaluation of syncope or long-term pharmacologic management of supraventricular tachycardia graphically outline reasonable approaches to patients with these problems. The illustrations are helpful; informative color plates in the middle of the book show how cardiac mapping helps in the localization of arrhythmias. The information in the book is accurate and up to date. Experts in electrophysiology give well-reasoned recommendations for evaluation and management, with appropriate suggestions for trials of medical treatment before consideration of ablation and prudent recommendations for ablation as the initial treatment in appropriate patients (e.g., patients with syncope and the Wolff-Parkinson-White syndrome). One might quibble with the recommendation for early use of an implantable loop recorder for the diagnosis of unexplained syncope, when results on tilt-table testing would be diagnostic in many cases and less invasive. Topics related to supraventricular and ventricular arrhythmias are covered in detail, with no major omissions. The only meager area of the book is the coverage of permanent pacing, which receives only one chapter that is limited to a discussion of indications. Inevitably, there is some redundancy, which results from having several authors cover slightly different areas of the same general topic. For example, in the various chapters on supraventricular arrhythmias, mechanisms and treatment of supraventricular tachycardia are covered both in the chapter on diagnosis of supraventricular tachycardia and the chapter on catheter ablation of supraventricular tachycardia, since knowledge of mechanisms is essential for successful ablation. It is not easy to target the general cardiologist and the primary care physician successfully in the same book. Although neither group specializes in cardiac arrhythmias, the cardiologist is expected to be better versed in details of the diagnosis and management of arrhythmias than the primary care physician. There is, for example, a general introduction to the approach to the patient with palpitations, which is appropriate for the general practitioner. There are also descriptions of reentrant circuits in ventricular tachycardia, which are perhaps beyond the interest of most cardiologists who are not electrophysiologists. This book would be most appropriate for the physician with a basic understanding of cardiac arrhythmias who seeks a detailed understanding of recent advances in this field, particularly with regard to newer therapeutic techniques and options. A physician whose patient has a particular arrhythmia will find an adequate explanation of the mechanism of the problem and descriptions of the most advanced electrophysiological approaches to evaluation and management, along with a discussion of when referral to an electrophysiologist is appropriate and what to expect from that referral.

Challenges in Colorectal Cancer

Review:

Colorectal cancer presents some of the most challenging problems for basic scientists, clinical investigators, and practitioners. It will be important to facilitate an ongoing exchange among these three groups if we are to transform many small steps into a major advance in the treatment of colorectal cancer. Surgery remains the center of attention, for two reasons. First, in any high-risk case, the surgeon must coordinate the various treatment options (e.g., preoperative, intraoperative, and postoperative adjuvant treatment); second, the surgeon determines the prognosis, since inadequate surgical technique can influence a patient's outcome more than can the stage of the disease. But there are new trends, such as risk-adjusted screening, brought about by molecular techniques that have deepened our understanding of the pathogenesis of colorectal cancer. The demonstration of micrometastases in stage II and III cancers has sharpened the need for effective adjuvant therapy.

This book deals with these challenges and presents important new developments. A consensus has evolved with respect to diagnostic and therapeutic strategies in dominantly inherited colorectal cancers (familial adenomatous polyposis and hereditary nonpolyposis colorectal cancer); by contrast, the rules for the early diagnosis of and screening for sporadic cancer remain undefined. Screening of the general population is effective but costly.

It is increasingly evident that preoperative chemotherapy or the combination of radiation therapy and chemotherapy can improve results in the treatment of rectal cancer. Experience during the past 15 years has clearly demonstrated the value of chemotherapy in high-risk colorectal cancer. It will become necessary to change the usual postoperative treatment with radiation or a combination of radiation therapy and chemotherapy to a preoperative combination, perhaps with additional intraoperative irradiation followed by postoperative chemotherapy. It is still debatable whether the excellent results of total mesorectal excision in patients with rectal cancer eliminates the need for preoperative, intraoperative, or postoperative radiation therapy. Tumor that has spread to the lymph nodes along the mesenteric border can easily be removed surgically, but removal of tumor that has spread to the pelvic wall is not part of the standard surgical procedure. Extended surgical procedures lead to high rates of irreversible loss of bladder control and impaired sexual function.

A number of new chemotherapeutic agents (oxaliplatin, irinotecan, and topotecan) and several types of immunotherapy are promising and may have clinical applications, but only if minimal residual disease can be diagnosed reliably. Dormant tumor cells are a reality and can be treated with the monoclonal antibody 17-1A. In the end, however, the solution will probably be a combination of immunotherapy and chemotherapy.

More knowledge of molecular biology, better definition of risk groups, better screening, further development of risk-oriented combination therapy, and more meticulous surgery may yield much higher cure rates for colon and rectal cancer. Scholefield's book is one of the best starting points for a journey through the world of established, new, and evolving treatments for these diseases.

Wer hier mitarbeiten will, muss die verschiedenen physikalischen Ableitverfahren zumindest halbwegs verstanden haben. Bereits geringfügig andere Kurvenverläufe können erhebliche diagnostische Unterschiede bedeuten -- mit gravierenden Folgen für die Gesundheit des Patienten und die Moral von Isabel oder anderer junger Ärztinnen und Ärzte.

War es vor 15 Jahren noch selbstverständlich, dass man als Berufsanfänger während seiner Klinikjahre auch adäquat ausgebildet wurde, so ist dies durch strukturelle Verwerfungen heute fast eine Ausnahme. Unter dem Primat der Fallpauschalen, der Diagnosis-Related-Groups, zählt hoher Durchsatz an Patienten, die möglichst von den bereits fertig ausgebildeten Oberärzten zügig untersucht und behandelt werden, damit sie schnell wieder entlassen werden können. Für Ausbildung bleibt da kaum mehr Zeit.

Der EKG-Kurs für Isabel knüpft daher an ältere Zeiten an. Persönlich und kollegial in der Diktion, wenden sich in diesem Buch zwei erfahrene Ärzte an die jüngeren Kollegen und erläutern die praxisrelevanten Grundlagen und typischen EKG-Befunde. Den größten Teil des Buches bilden die 49 originalen Beispiel-EKG samt Beschreibung, Deutung und Kommentar. Das Buch ist in weiten Teilen weiß-rot -- wie das EKG-typische Millimeterpapier. Man sieht vor seinem geistigen Auge den erfahrenen Arzt, wie er am Krankenblatt EKG und Befund erklärt. Zu guter Letzt finden sich unkommentierte EKG und Quizfragen samt Lösungen. An ihnen kann Isabel -- und jeder andere Berufsanfänger -- das Gelernte überprüfen.

Dieser EKG-Kurs in dieser persönlichen Ansprache ist ein hervorragendes Buch für junge Mediziner in schwierigen Zeiten. Man wünschte sich, er wäre nicht nötig. Aber es ist gut, dass es ihn gibt. --Dr. Stefan Rusche
